are you sure it was a worm? Could it have been a mantis shrimp?

ok ive been searching for the ID of this worm everywhere but i cannot find it..
it was about 6 inches long with a red head area about 1/2 inch long with a maw. One night i went to bed and everything was fine the next morning my yellow tang had a hole about 1/8 inch diameter strait though it.. i searched everything in the tank looking for a cause but found nothing.
about a week later another fish had the same.. one night while watching the tank in the dark i saw it and watched where it went but never could catch it.. after it killed 1 more fish i had to dismantle my tank inevitably killing everything... i would really like to find out what this was so when my new tank gets set up i know what region of live rock to be suspicious of.

That thing sound viscous...Do you think that the fish are dying from another source and the worm is being opportunistic?
no its not a mantis shrimp... it may be that it turns out to be a eunice type worm..i dont have pics i didnt have a camera at the time... and it was the worm attacking the fish i saw him attack the 3rd fish and i tried to catch him at that time but couldnt and that night he killed the fish so i ended up having to purge the rocks which killed all my inverts
yeah i eventually got him but am having to start all over again.. so ive come lookin for advice and others know-how.. this place is great..
